Our Home Health care team works to help you recover at the comfort of your home, living as independently as possible. We work with your primary care physician to develop an individual plan that is centered around the priorities and wishes of the patients and their loved ones.

Nurse Case Manager

A Nurse Case Manager will make regularly scheduled visits assessing pain and providing symptom management techniques. The nurse will educate patients and their families about disease progression, use of medications and daily care needs.

Home Health Aide

Home health aides assist patients with activities of daily living. Activities most often include bathing, dressing and a variety of other personal care services.

Social Worker

Our social workers have specialized training addressing the practical and financial needs of each patient. This may include emotional support, counseling and serving as a liaison to community agencies.

Physical Therapist

Our physical therapists work with you to restore balance, and improve mobility, all in the comfort of your own home. Services may include home safety education, fall prevention, and home exercises.

Occupational Therapist

Our occupational therapists can help improve muscle strength needed for activities of daily living. They are also available to educated patients and families on the use of any adaptive equipment.

Speech Therapist

Speech therapists are available to provide intervention for patients that are having problems with impaired speech, eating and swallowing difficulties, communication obstacles and other problem areas.
